For Team Manager's to fill out in the event of an injury

To be filled out & submitted to the BDJFA prior to a player wearing headwear.

The Dunsborough Football Club carries medical insurance for all registered players to assist with the costs should a player need medical treatment as a result of an on-field injury - be it during a game or during training.

Our Platinum level insurance is arranged through JLT Sport AFL. Any member from our club who wishes to make a claim should first visit JLT's website where they will find full details of the insurance cover and where they can download a claim form.
